What Is Xeomin?

Xeomin is a neuromuscular blocking agent that is indicated for the improvement of certain muscle disorders in adults. It contains botulinum toxin type A, a type of toxin purified from the bacterium Clostridium botulinum and free from other proteins. This FDA-approved agent inhibits acetylcholine to relax the muscles. It is administered by injecting reconstituted units intramuscularly.

Areas Treated with Xeomin

  • Upper limb spasticity –  For adult patients who experience severe spasticity or local muscle weakness, such as clenched fist, fixed elbow, and thumb-in-palm syndrome, this toxin can help ease symptoms. Each condition requires a different dosage and number of injection sites per muscle, with an average of 25 units – 100 units.
  • Cervical dystonia – Adults experiencing involuntarily neck muscle contractions and an abnormal head position can receive treatment with this neurotoxin to relax the affected muscles. It is applicable for both previously treated and new patients. The recommended dose is 120 units of Xeomin.
  • Blepharospasm – Xeomin is used to treat this condition in patients previously treated with Botox. The initial dose for each eye should not exceed 70 units.

Suitable candidates

Adult patients who wish to look for an effective treatment for severe muscle pain caused by conditions, such as upper limb spasticity, blepharospasm, and cervical dystonia, are good candidates for Xeomin.  

This is not recommended for patients who are allergic to botulinum toxin type A or who have a skin infection at the planned injection site. Patients should also inform the doctor if they have problems breathing, bleeding disorders, or are scheduled for a surgery. Xeomin is not recommended for treatment in pregnant or breastfeeding patients. Discuss whether your patients are pregnant or are planning to become pregnant prior to treatment.

Xeomin is not recommended in pediatric patients.

Use instructions

Xeomin is intended for intramuscular injection. You may wish to do a patch test prior to the treatment to determine suitability. Mark the proposed injection sites with a pen. Inject into the muscle and avoid other sensitive structures.

Different sites and indications require specific doses for optimal effects. For example, upper limb spasticity and cervical dystonia may require 25–120 units, depending on the severity and the location. The whole procedure may take an average of 30 minutes. Patients can go home immediately after the procedure and do not require additional recovery time. This toxin is not to be used with other botulinum toxin brands.

How does it work?

The neurotoxin is an active neuromuscular agent that inhibits the release of the neurotransmitter acetylcholine into the synaptic cleft. The temporary muscle paralysis allows the muscle to relax and reduces painful contractions. Xeomin is also available as Xeomin Cosmetic for the correction of forehead lines.
This botulinum toxin is free from any complexing proteins, which minimizes the amount of foreign proteins while exhibiting high efficacy and safety.

When should my patient come back for treatment?

Each indication and individual may exhibit a different duration of effectiveness. Some individuals may experience a significantly longer or shorter effect. For aesthetic purposes, the smoothening benefit may last up to 3 to 4 months. Follow up or maintenance treatment is necessary.

For severe spasticity and other muscle disorders, the muscle relaxation may last up to 10 to 48 weeks, depending on the amount of neurotoxin units delivered.

Reported side effects

Although it is safe and effective to undergo botulinum toxin injections, it can cause some side effects. If this toxin is administered by an unlicensed injector there is a risk of the spread of the toxin.

Common reactions:

  • Dizziness
  • Dry eyes
  • Neck pain
  • Injection-related reactions, including redness, swelling, and pain at the injection site
  • Headache
  • Eyelid bruising or swelling

Tell your patients to seek immediate medical attention if they are experiencing problems with swallowing, speaking, or breathing after an injection of Xeomin.

Although they can resume to their daily routine after the procedure, it is strongly advised to avoid strenuous exercises or operate heavy machinery. Patients should report any prolonged side effects to you. For all side effects specific to the area of treatment, refer to the product pamphlet.  

Cost of treatment

Xeomin and other botulinum toxin injections are priced according to the units used or at a flat price. You can determine the cost of this treatment in your clinic based on the number of units needed and the number of areas being treated. Compared to Botox, which can cost up to $15 per unit, Xeomin can slightly more cost-effective. Some doctors charge the same price for both brands.
